Can anyone recommend programs for collecting customer information for a newsletter?

You mean the opt in forms and then email list management?
I use iContact for my newsletters.

Very inexpensive. Quick and easy for creating and sending newsletters too.
Here's my opt-in form for newsletter subscribers. Very simple.
You can probably make it fancy, but mine is just basic.
